Sounds like a mail database issue.
Try manually running the database repair and see if that fixes it. See @Gary post below in the following thread on how to do that for Pc or Mac.
Now if manually repairing the database still won’t fix the database sync error, If you have an IMAP, Exchange, Office 365 or iCloud account “and nothing normally kept in eM Client Local folders”, I would then suggest to go to the following eM Client hidden database folder below, and rename that to eg: eM Client-Old. Then open eM Client and setup your account/s as new and all your emails, calendar. contacts etc will come back from the server when eM Client syncs.

(Hidden mail database folder Mac)
“Users\yourusername\Library\Application Support\eM Client”
Now if you had a POP account and / or you had messages in Local folders, then “if you had been making regular eM Client backups” either automatically via Settings or manually via the menu, then after renaming eM Client database, re-setup your POP account and then restore your eM Client backup via “Menu / File / Restore”.
Now if you had a POP account “and didn’t have any eM Client recent backups”, then “if you have been regularly backing up your OS user profile”, then copy the contents of the eM Client hidden mail database folder contents from your backup and then delete your current hidden folder contents and then replace it with your backup version. Then open eM Client.
Lastly once you get back into eM Client, check that you are running the latest V8.2.1721. If you don’t have that version, you can download that via the release history page and update your current version. Close eM Client before updating. Also make a manual backup via “Menu / Backup” before updating.
